Increased Pressure giving it more velocity (punching power)

You can also get +P+ but you do have to be careful as certain guns (cheaper models) cannot handle it.

If you have a Glock (for example) you can shoot whatever you want as those things are built for abuse.

Generally, the owner's manual for your firearm will describe whether or not +P loads can be used. If in doubt contact the manufacturer. Using +P in a firearm not designed for the increased pressures can and will damage the firearm. Damage ranges from stuck slides to bulging or even exploded chambers. Barrel wear is increased regardless but better designs will handle the wear better.

How do they achieve this additional pressure?

By "hot loading" the bullet or is it something else that is done during fabrication?

This was my understanding but I certainly have no authority on the subject. Some handgun ammunition will be of the same manufacturer and with the same number of grains but one will be rated +P and will deliver higher muzzle velocities. This is readily explained by a faster burning powder that will induce a higher pressure before the bullet exits the barrel. If +P only designated more grains of powder it would be a useless metric as grain count is already reported for any load.
